Application Form

To apply for Brooke House College please complete the digital registration form below, along with the supporting documentation. 

Supporting Documentation:

• School reports
• Transcripts of Qualifications obtained and courses taken, if applicable
• Copy passport for Mother
• Copy passport for Father
• Scanned copy of your Passport (student)
• Birth Certificate of applicant – translated if applicable
• Football Player CV Template | PDF, if applicable
• Financial evidence - Please note funds should be available in a personal bank account in either of the parent or guardians name for at least 28 days. 
• Admin fee of £500 + VAT - Paid online via FlyWire, either with a credit card or a bank transfer.
• Official Examination results, if applicable
• Reference from previous or current school
